Before filling |
Print out the
printed order form page so you can take it with you to measure your pet and record the
measurements and other information to fill in on our online order page. Fill in all blank lines that apply. Return to the Online Order Form page. Transfer information from this printed form to our online order form. You may choose to fax or mail us the order instead of sending it to us via the World Wide Web. If you fax it be sure to include a form of payment: Visa, MasterCard, or Discover card number, expiration date, and name of person on card. If you mail the forms to us you should include a check or money order, or a credit card number. If you are reluctant to send your credit card number over the internet, then just type "Calling with card number" in the credit card number field on the online order form.
